Program Description
The Jay home is located within walking distance of downtown Livermore Falls. This four-unit apartment building is designed for three individuals with intellectual/developmental disabilities. Each consumer has their own apartment. Consumers are staffed 24-7 with 2 staff on at all times.
The direct care staff assists consumers with skill building, social skills, independent living, and community inclusion. Staff may also assist with medication management, behavior management, supervision while in the community, cooking, grocery shopping, cleaning, and organizing, as well as coordinating outside service provider appointments and day program services.
